SABE Boxing Consent & Participation Form

Thank you for your interest in SABE's free boxing classes.

Please complete this form to register your child, or yourself if you are aged 16 or over. We ask that you provide the necessary information and confirm your consent preferences. Your responses help us deliver a safe, inclusive and enjoyable boxing experience for all participants, while supporting the continued development and future funding of our free boxing programme.
